Receiving your order by a truck (LTL) - what to do DURING delivery

Please review the following procedure before your delivery.

The customers or person receiving the order is responsible for documenting any loss or damages that might result from the carriage and delivery of freight. If the receiver fails to document the loss or damages or fails to do so in a prompt and appropriate manner, then the manufacture or Wellavita Inc will not be responsible for any damage or losses.

Properly inspect all incoming items at the time of delivery.
- Verify that all materials are complete.
- Check the number of boxes or pallets received against the bill of lading.
- Visually inspect for damage.

If all is acceptable.
- Accept delivery. Sign and Date.

If there is concealed damage after delivery has been accepted.
The customer has 48 hours to report concealed damage after they originally signed the bill of lading in good condition. The manufacture will be unable to file a claim with the carrier after the 48-hour period has expired. The customer will become responsible for all damage.


- Photograph immediately so you will have a visual record.
- Report damaged or missing items immediately to info@wellavita.com.
- Send order number, photographs of damaged items, and a copy of the signed bill of lading with the date and hour of when items were received.

If the shipment is NOT complete
- Note on freight bill “INCOMPLETE” and the number of items missing.
- Report missing items immediately to info@wellavita.com.
- Send order number, photographs of damaged items, and a copy of the signed bill of lading with the date and hour of when items were received.

If the shipment is DAMAGED.
- Photograph immediately so you will have a visual record.
- Determine if the damage is sufficient to refuse the shipment or if you wish to repair the items in the house with replacement parts. Even if you decide to accept the shipment in damaged condition – note “DAMAGED” on the freight bill.
- Report damaged or missing items immediately to info@wellavita.com.
- Send order number, photographs of damaged items, and a copy of the signed bill of lading with the date and hour of when items were received.

If the shipment is SEVERELY DAMAGED.
- Photograph items prior to return.
- Refuse shipment
- Advise driver that goods must be returned to the manufacturer.
- Report damaged or missing items immediately to info@wellavita.com.
- Send order number, photographs of damaged items, and copy of the signed bill of lading with date and hour of when items were refused when signing.
